Reminders for Principal Investigators
- Complete the "Protocol Details" section of your protocol before assigning a new Team Memeber/Study Staff to it (My Protocols > Protocol Details).
- Use the Search feature to add a new Team Memebers/Study Staff to your protocol. If the user is missing from the retunred list, please request their name to be added using this form: myResearch Grants - Access Request Form.
I submitted my health questionnaire. Why have I not received a clearance notification?
- In addition to submitting a health questionnaire, a tetanus vaccine record within the past 10 years is required for clearance. Please make sure you submit a tetanus vaccine record from within the past 10 years (e.g. Td or Tdap vaccine) to ndlar-docs@stonybrookmedicine.edu so that your health questionnaire can be reviewed. If you need to make an appointment for a tetanus vaccine, please call the Stony Brook Occupational Medicine clinic at 631-444-6250 and identify yourself as a Non DLAR researcher.
- If your research protocol requires the use of an N95 respirator, you will also need to submit a respirator health questionnaire, which you can access from the nondlar app.
- If your research involves work with Mycobacterium tuberculosis, you will need to submit tuberculosis screening tests. If you want to make an appointment for a tuberculosis screening test, please call the Occupational Medicine clinic at 631-444-6250 and identify yourself as a Non DLAR researcher.
